I donβt think it is a surprise to anyone that increased turnover of staff is associated with poorer outcomes for patients. I sent this to my administrators. Staff and physician retention are good for patient care.

Could I suggest there may be selection bias? I donβt have the full manuscript to review but itβs βadvanced PDβ and likely a selection bias for levodopa responsive PD. If the tremor doesnβt respond are they really PD and culled out from being βadvanced PDβ.
As a βruralβ neurologist (Fargo) I can relate to this. Many of my patients come from quite a distance (Minot to Dickinson to South Dakota) and even Winnipeg at times.
www.ajnr.org/content/45/1.... I donβt believe itβs a just a radiological diagnosis. There are many people with abrupt onset incapacitating orthostatic headache without radiologic findings who fit the phenotype.
This is one of the most difficult things to get treated. I used to be able to get my colleagues to do empiric blood patches within days of onset of orthostatic headache, now they want exact localization of the leak which delays early intervention.
