Yes, I treat it as if I’m doing a de novo ablation in a patient with longstanding persistent AF. Start with the veins and the posterior wall and take it from there.
For #Epeeps who are doing all PFA for afib, have you had patients with refractory afib after multiple RF ablations who you’ve had meaningful success after PFA? If so what was your approach?

Yes! Delivering PFA over thermally ablated cardiac tissue with no electrically signals in patients with refractory AF have benefitted many of my patients.

I think we have most bases covered in this VT entrainment talk (with these being the culminating slides after lots and lots of preceding foundational material about interpreting egms and pacing in and around regions of scar during VT and during sinus)

Tough transseptal due to septal aneurysm, PFO, and thick limbus with a narrow window. Had to shepherd’s hook it in the LA to position the catheter in the RIPV. #epeeps

Congratulations to Lena Rivard on the @ahascience.bsky.social presentation of the BRAIN-AF study results.
Major observation is that cognitive decline occurs in many « low risk » AF patients.
Unfortunately, Rivaroxaban 15 mg daily did not impact this risk in this double-dummy RCT.
Congratulations to @omwazni and the OPTION study team on the @ahascience.bsky.social presentation and publication of the OPTION trial.
Similar low rates of stroke post ablation (no difference with laao vs OAC) with no significant 23% lower rate of major bleeding.
