Because she entered an ENT residency to become an ENT surgeon. It’s only after she quit surgery that she pursued other things. I don’t think there even is a Functional Medicine residency program.

More pertinently you can say she is not a fully trained or board certified physician. Anyone who graduates med school is a physician but they are not ready to take care of patients until after completing training and certification. She never did either.
