True! I have a whole set of associations with the phrase that overlaps with my parents' attitude towards medical care in general (they avoided it, and stayed healthy until they didn't), so my use of the phrase is in the ballpark of how Brits use it but might be idiosyncratic.
I am, and I do now and then, but only because I picked it up from my parents (born in the UK, moved to the States before I was born). I more often heard it as "in rude good health."

I spent some time with a random sample of fifth graders this week, during which I discovered they knew exactly two things about Star Trek: 1. The Spock hand gesture. 2. "George Takei was in it."
When I lived in the financial district of NYC, I started referring to anything north of Houston St. as upstate.
I was at a showing of The Birds where the audience got the giggles early on because of Tippi Hedren walking on the beach in high heels, and then everything seemed ridiculous instead of scary. Not ideal, even though I understood how it happened.
