A recent RCT published in @jamanetworkopen.com finds #pharmacist-led outreach guided by an evidence-based hypoglycemia-prevention algorithm was more likely to lead to safer diabetes prescribing compared to a control group.

#deprescribing needs careful monitoring. This week, restarted medication (albeit at lower doses than originally taking) for 2 patients whose symptoms worsened after stopping. A deprescribing success - found out the medication likely helping more than harming; now will assess response to confirm.

Is anyone studying effects of these drugs in frail older people and effects of #deprescribing ??
