I think the point the authors end with is worth highlighting. In this era, I don't think it matters if a traditional journal chooses not to publish it. It will just get released in some other form with some claiming censorship or defaulting to the shill gambit.

That makes a lot of sense. Much respect to you and your colleagues for practicing medicine in that setting. Definitely spoiled at an academic medical center.

We are lucky to be able to dispense an epinephrine auto injector kit in the ED prior to discharge.

Unfortunately, there’s been times when an admitted patient just ends up staying in the ED until they got better. Never made it to a ward or short stay unit because the hospital is so full. I’ve typically seen around 4 hours observation after anaphylaxis assuming no other complications.

Thanks for commenting. 12 hours feels like a long time. Would you mind sharing some background on the setting you practice? It feels like that length of observation time where I practice may worsen the overcrowding and boarding situation in the ED.

Pharmacokinetics and pharmacodynamics following repeat dosing of neffy (epinephrine nasal spray) versus intramuscular injection during induced allergic rhinitis - PubMed Repeated dosing of neffy, including during NAC-induced allergic rhinitis, yields PK/PD profiles comparable or greater to IM epinephrine, supports same-nostril repeat dosing, and provides a well-tolerated, needle-free option for patients who may require a second dose.

I found a study talking about the pharmacodynamics pharmacokinetics comparing intranasal with IM. Don’t see many studies with patient-oriented outcomes at this time. pubmed.ncbi.nlm.nih.gov/41687867/

I’m assuming you are referring to the intranasal epi? I don’t know if there’s much data to answer your question at this time. It’s relatively new and I still find most patients and families I encounter have the IM autoinjector.
