Yeah what I mean by not having 2+1, is still having things like median barriers, wider shoulders etc, just not always having passing lanes always (ie like what happened with Dome Valley)!

That's one potential option! Might not need the full 2+1 everywhere, but along those lines.

It depends on the context, but the point about 'right sizing', is looking at what you actually need rather than imposing four lane grade separation everywhere!
