Before, we've stuck to central lines for pressors to avoid extravasation but waiting for CL isn't an option when profoundly hypotensive. Short-term peripheral admin of pressors is safe. Complications arise after ~55h of infusion

Antidote you ask? Yes, DigiFAB can be used, if indicated (e.g. random total level ≥15 ng/mL or ≥10 ng/mL 6 h postingestion, arrhythmias, etc)

Dialysis? Usually not a great option since dig has a large Vd (4–10 L/kg)

Patients can present with 🤕,🤢, 🤮 and/or
♥️ manifestations!
♥️🕐 Early on increased vagal tone causes AV nodal blockade

⚡ Lytes! Toxicity typically causes hyperK (but hypoK increases toxic effects)

I’ve seen EPIC reports for running antibiotic data (can process real time orders) compared to SlicerDicer. The reports work well if you work in an ED that does only 1x med orders but the most efficient way would be a dashboard & maybe something that catches bug-drug mismatch w/ previous cx data IMO!

We do not, but definitely a great idea and likely used on inpt side! We make most stewardship interventions when we attend ‘rounds’ which is essentially provider pass off on our most acute ED patients but this is only ~30-42 out of 220-250 patients based on time of day so a dashboard would be great!
