One template does not fit all: where next to improve hospital discharge communication to primary care? | Primary Health Care Research & Development | Cambridge Core One template does not fit all: where next to improve hospital discharge communication to primary care? - Volume 26

Have you ever read or written a hospital discharge summary? Did it serve its purpose? Did it need more than just β€˜the standard information’? Did the template help? Incredible Izzy has just passed the first stage of her hearing dog training - the Puppy 1 Star ⭐

Izzy has been working on her basic cues such as 'sit' and 'wait', and has also been encouraged to investigate lots of new sights, sounds and smells.

Congratulations 'Team Izzy' πŸΎπŸŽ‰
