Oh yeah, in addition to Pokemon Day, today was Americaβs national resident and fellow appreciation day. We got a cute message from the hospital administration, a cake from our clinic faculty, and some balloons from our staff and nurses. βΊοΈ

I just gave her some pamphlets for support groups, advised her to keep thinking about the rehab thing, let her know her situation is a little worse than a few months ago, and talked to the people on the hospital team about it⦠I asked them to bring up rehab some more before she gets discharged.
Another option is comfort care. We can try to qualify for hospice, get pain under control, and stop trying to work so hard to get a liver transplant. β¦ The patient doesnβt want that either.
The thing is, since weβve tried the other option so many times, I donβt think thereβs a good chance it will work by itself, which is why I think rehab is best, but I canβt force anyone to do that. It has to be their choice.
In this case, I tried to make a case for voluntary rehab, saying I strongly believe this is the best chance for survival, and they said no. β¦ So I said the next best alternative (something weβve tried several times now) is abstinence and a sobriety fellowship group.
I believe that as doctors it is our duty to provide patients with the information we gather, involve them in our thought process, both in ways they can understand, so they can have ownership of their health decisions. I never βtalk shopβ around patients.
Specifically this person has some end-stage problems relating to alcohol, and they need a transplant sometime soon to have a chance at life. β¦ I went to their room on my day off to talk about goals of care.
Iβm not on my hospital rotation this block, but I keep an eye on the list for interesting cases or whenever I have my own clinic patients admitted. This is my #1 frequent flyer, and sadly things are not looking great overall.
