I don't think that fits with the placebo effect, which generally leads to modest changes in self-reported measures. It's also a much bigger effect than we've seen in numerous clinical trials.
Pace explicitly tested the fear/avoidance model, no gain in fitness. Minimal in walking.

I think shows the placebo effect is pretty weak, and is more about response biased a questionnaire then real change. E.g. with diabetes, you might get a modest open (subjective) questionnaire score shift, but not a change in (objective) blood insulin or glucose levels.

1) A new Norwegian study tested more than 6000 proteins in the serum using aptamers (small pieces of DNA or RNA that can bind with specific targets such as proteins).

751 proteins showed a significant difference between ME/CFS patients and controls.

Same old story from psychosocial researchers.
An unblinded CBT trial found modest gains in the subjective outcome of fatigue. Which is almost likely bias. They measured physical activity with wearables, but failed to publish the results, later acknowledging no gains.
So no evidence CBT works.

The start of my illness, back in 1996 – the UK medical Royal colleges wrote a report with similar recommendations. I had assumed that they would do a good job - they were doctors in Royal colleges. 30 years on, we are still in the same barren place.

Which is a lousy definition of PEM. Where are the disease controls, such as MS and rheumatoid arthritis – I suspect they would have a significant proportion meeting these criteria.

Thanks. I agree that the 50% figure is unreliable. I'm not sure how reliable the 20% figure is either eg the ONS figure uses a very broad implementation of LC. I'm not sure the long Covid field has got a robust case definition yet: Hard to estimate the rate of ME/CFS without one.

It is, but I think it's very important the field tries to replicate such findings. Otherwise, we end up with lots of promising results pointing in different directions, and no obvious way forward
