My bet is skin cancer (decades of golf), and the cream is being used to treat/prevent it from spreading. I know that there's a cream made for that reason that does cause reactions like that.
Considering the amount of time he's spent on a golf course, I'd say skin cancer is a strong possibility, even in that spot. There's a cream treatment that's been mentioned in this thread that can cause some painful-looking rashes.
