If you run something in the background, and it then stops constantly to ask for approval: 1. you don't have the context to approve the request (especially for write operations), 2. it doesn't seem to use the previously approved list of commands in settings.json. 3. the agent isn't really background.
I use Claude Code for personal use with the Pro plan. For work, I have Copilot and Codex. I haven't used Codex in awhile, but Copilot, at least in VS Code chat, keeps changing its default behavior (now preferring background agents) or ignoring command approvals.
